Eleven weighted predictors (age plus male sex, cancer, heart failure, chronic lung disease, tachycardia, hypotension, tachypnoea, hypothermia, altered mental status, hypoxaemia) define Classes I–V; 30-day mortality ranged from ~0% (Class I) to ~25% (Class V).

The 2019 ESC pulmonary embolism guideline uses PESI Class I–II (or sPESI 0) to identify low-risk patients who may be candidates for early discharge or home treatment, provided there is no right-ventricular dysfunction or raised troponin.
