AHA PREVENT — 10-year ASCVD, cardiovascular and heart-failure risk, ages 30–79
For adults 30–79 years without known cardiovascular disease. Valid ranges: total cholesterol 130–320 mg/dL (3.4–8.3 mmol/L), HDL 20–100 mg/dL (0.5–2.6 mmol/L), systolic BP 90–200 mmHg, BMI 18.5–39.9 kg/m², eGFR 15–150 mL/min/1.73 m². The equations do not use race.

When to use
Estimates the 10-year risk of total cardiovascular disease, ASCVD and heart failure, and the 30-year risk, in adults 30–79 years without known cardiovascular disease; the models are sex-specific, race-free and include kidney function (eGFR). [1]
Pearls and pitfalls
• The equations were derived in 3,281,919 adults from 25 datasets and validated in 3,330,085 adults from 21 further datasets; in external validation, the median C-statistic for total CVD was 0.794 in women and 0.757 in men. [1]
• The 2026 ACC/AHA dyslipidemia guideline adopted 10-year PREVENT-ASCVD as the preferred primary-prevention risk estimate, with categories of low (< 3%), borderline (3 to < 5%), intermediate (5 to < 10%) and high (≥ 10%). [2, 3]
• PREVENT gives lower absolute risk estimates than the Pooled Cohort Equations; in a US survey sample, the full 2026 framework nonetheless raised statin eligibility from 39.8% to 54.3% of adults aged 30–79, most among those under 50. [4]
• The equations cover total cholesterol 130–320 mg/dL, HDL 20–100 mg/dL, systolic BP 90–200 mmHg, BMI 18.5–39.9 kg/m² and eGFR 15–150; values outside these ranges are outside the model. [1]
