Evaluation of pretest clinical score (4 T’s) for the diagnosis of heparin-induced thrombocytopenia in two clinical management studies. A low score (0–3) had a negative predictive value near 100% for HIT; intermediate (4–5) and high (6–8) scores carried progressively higher probability.

Guideline
The 2018 ASH guidelines on the management of HIT recommend the 4Ts score to stratify pretest probability, reserving immunoassays and functional testing for intermediate- or high-probability patients.
